Prior to your intake appointment, please print and complete the following and bring to our first session:

Read the Privacy Statement and Informed Consent for Treatment

Print and sign the final page of the Informed Consent for Treatment (one copy for each person being seen)

Print and complete the Client Information and Psychosocial History forms (one for each person being seen)
